Engineering Leadership: The Golden Rule

A critical rule in matrix management (and UK-SPEC Competence C) is that for any given task, there must be only one "Accountable" person (The 'A'). This is the individual who signs off the work and holds the ultimate risk. While multiple engineers can be "Responsible" (The 'R') for doing the work, splitting Accountability leads to decision paralysis.
